IMPORTANT NOTICE RE ST CLEMENTS SURGERY:

In order to utilise our resourses more efficiently we will closing St Clements Surgery on 2 afternoons per week.

From 6th August 2009, it will be closed on both Wednesday and Thursday afternoons.

From 30th November 2009 the surgery will be operating a new appointment system.

We have decided to change the way appointments are made to help with the expected increase in demand during the flu' season, to try to reduce the number of DNAs at the surgery and to provide better continuity of care between you and your Doctor or Nurse Practitioner.

From the end of November you will be able to book your appointment with a Doctor or Nurse Practitioner on the day that you need to be seen (if we are very busy and your problem is not urgent you may have to wait until the next day).  As all Doctors' appointments will be "same-day" it means you will not be able to forward book an appointment. Appointments in the future (2-6 weeks in advance) will be booked by the Doctors, if they wish to follow you up. 

We expect the telephones to be very busy at the start of the new system(particularly on Monday and Tuesday) so please be patient or ring for your appointment later in the week.

Please note that you will not be able to book an appointment via the website.

Terminally ill patients can contact the practice at any time for advice, help with medication, or to organise an appointment with a doctor or nurse.

Making An Appointment To See A Nurse

Routine appointments are usually available with a nurse or Healcare Assisitant within 2 weeks and sometimes sooner. Phlebotomy (blood) appointments are offered only before 11am  to meet the delivery service to the hospital laboratory.  You can arrange a routine nurse appointment at any reception desk or by phone after 10.30am each weekday.  Please be ready to tell us exactly why you need the appointmant so that we can make sure the nurse has sufficient time to see you.

You may receive a letter asking you to attend a specialist clinic (Diabetes, Heart Health etc).  If you cannot keep the appointment please telehone the number given in the letter and make arrangements for another day.

WE DO NOT OPERATE A WALK IN SURGERY.  You will need a pre-booked appointment to see one of our clinicians.  If you just turn up at the surgery without an appointment you will be disappointed.

Home Visits

Home Visits are for those patients who are housebound or who are genuinely too ill to attend the surgery and you may be asked to attend surgery if you are able.  Lack of transport is not a reason to request a home visit . If you request a non-urgent visit after 12noon, you may be asked to contact us the following working day

Bear in mind that in the time it takes a doctor to visit a patient at home, four patients could have been seen at the Practice.

Please telephone the surgery between 8am and 12noon with your request for a visit.

Serious Medical Emergencies such as severe chest pain, fitting, collapse, unconciousness, heavy bleeding etc should be directed immediately to the
999 Ambulance Service
