Medication authorised by the doctor for repeat prescribing, can be re-ordered by:

• filling in the form below
• by telephone (For housebound patients only)
• by post (Please enclose S.A.E. if you want your prescription posted to you)
• by calling personally into the surgery with the  re-order portion of your prescription.

Note:  We do not operate a precribing line for reasons of safety.  With the exception of housebound patients, we actively discourage the ordering of prescriptions by telephone.

TO MEET PATIENT SAFETY REQUIREMENTS YOU WILL NEED AN APPOINTMENT IF YOU REQUIRE MEDICATION WHICH IS NOT ON REPEAT OR  HAS NEVER BEFORE BEEN PRESCRIBED BY US.

It is nationally agreed that a minimum of two working days notice is required when ordering prescriptions.

It is the responsibility of the patient to ensure they have an adequate supply of their medication.

BEFORE ORDERING YOUR PRESCRIPTION PLEASE NOTE:

The amount of medication supplied may be governed by Hywel Dda Health Board and/or GP guidelines. You may not necessarily receive the amount you requested.
