Patient Registration

A patient joining the surgery will need to fill out the attached form and the GMS 1 form and provide proof of identity.

1 of the following:

 

A valid UK Passport   A birth certificate
A valid UK Driving licence with pictureA valid non-UK passport

(Non-UK nationals will be unable to register at the surgery without a valid passport).

If none of the above is available for a UK resident,two of the following, from the list below, must be used as a form of identification. If current address is different from address shown on evidence from list above, one of the following forms, showing correct address, must be provided. All will be photocopied by the practice.

 

NHS CardRental Agreement
Utility Bill or Mobile Contract Bank statement
Electoral role letter or polling cardSocial Services letter

 

 

 

All patients will need to provide details of their previous doctor and surgery address.

If a foreign national – the date of entry to the UK must be given.

All new patients will also be asked to complete a health questionnaire which is available from either of our reception desks or can be printed off from the link below.

 

This will allow the practice to collect information on your past medical health prior to receiving your full medical records. 

If you wish to register, please print off ALL of the forms below, complete them and return them to the surgery IN PERSON with the relevant documentation as requested.
